Before beginning the protection order process, please read the following information:

Sexual Assault Protection Order Forms

Each of the Protection Order types requires that three forms be completed. The forms and the instructions to complete them are listed below:

Modification

If you currently have an active Sexual Assault Protection Order from the court and would like to change one or more of the protections that you were originally granted by the judge, complete and file the “Request for Modification” below:

Renewal (Available soon)

If you currently have an active Sexual Assault Protection Order from the court, and it is more than 45 days before the expiration date of the original or otherwise modified protection order, and you would like to renew the order for one year, complete the forms below:

  • Praecipe (DC 19:1), Instructions (DC 19:1(a)) — The praecipe is how you ask the court to give notice to the respondent (the person you are seeking continued protection from).
  • Social Security Information Form (DC 6:5(12)), Instructions (DC 6:5(12a)) — This form provides confidential social security information to be kept in a private section of the court file.
  • Petition and Affidavit to Renew Sexual Assault Protection Order (Not yet available) — The petition and affidavit to renew is how you request the current, active protection order be renewed and provide details about why you are asking for the renewal to the court.

Dismissal

If you have an active Sexual Assault Protection Order from the court and would like to have the order dismissed (canceled), complete and file the “Motion to Dismiss” using the form below:
