November 2020 Form Revisions and Additions
The following forms were added or improved in November, 2020:
Guardianship/Conservatorship forms
Suggestions for revisions from judges, attorneys, court staff and the public resulted in changes to the following forms:
- Financial Institution Receipt of Orders, CC 16:2.6, and Financial Institution Receipt of Letters, CC 16:2.6.1, forms added the language, “The printout should include only the last 4 digits of the account and should be redacted to remove any personal identifying information (SS#, DOB, full account #)." as an additional reminder of the importance to redact confidential information.
District Court
- Court Created Parenting Plan, DC 6:5.7, the language in number 11 pertaining to summer parenting time was revised for accuracy and clarity. This required changing “During this period of time, the custodial parent is entitled to the same parenting time the noncustodial parent would have if the child(ren) was/were with the noncustodial parent” to “During this period of time, the custodial parent is entitled to the same parenting time the noncustodial parent would have if the child(ren) was/were with the custodial parent.”
Administrative/HR
- Vacation Leave Carryover Request, HR 3:11, was created by HR and uploaded to the intranet.
Supreme Court Appendices
- Report of Cases Under Advisement, Ch5Art3AppB. The Supreme Court adopted the revised automated form that includes all case types, the ability to add pages for reporting more than one case, and a submit function that directs the completed form to a specified email address. This appendix is effective 12/01/2020.
- The version without the introduction page is found on the Master Forms Listing as Report of Cases Under Advisement (All case types), Ch5Art3AppB_1.
- The individual reports specific to District Court and WCC, Ch5Art3AppB_2 and Ch5Art3AppB_3 have been decommissioned as all case types are now included on the one report.
