December 2020 Form Revisions and Additions
The following forms were added or improved in December 2020:
County Court and District Court
- Petition to Set Aside Criminal Conviction, CC 6:11, added “community service” and “not over 1 year imprisonment” in the "sentenced to" statements per LB881.
- Order Setting Aside a Criminal Conviction, CC 6:11.2, added statement regarding consulting an attorney on the defendant's ability to possess a firearm under state or federal law per LB881 requirement.
- Voir Dire, CC 7:3; Jury List, CC 7:4; and Jury Fee List, CC 7:7. The District Court Procedures Manual Committee requested that these three forms be revised for use by the district courts as well as the county courts. To accomplish this, the option to choose the court type in the header is made available.
- Praecipe for Subpoena, CC 4:6, added checkboxes and instructions to designate if a witness is an Employee of the State of Nebraska, a political subdivision or a privately-employed security guard pursuant to Neb. Rev. Stat. § 25-1223(6). This is added to provide information for the clerks accepting the requests.
Supreme Court Appendices
- Order Sealing Exhibit(s) Pursuant to Rule § 6-506 and § 6-507, Ch6Art5App1, the Supreme Court adopted this new appendix 11/25/2020.
- Order on Public Review of Exhibit(s) Pursuant to Rule § 6-507, Ch6Art5App2, the Supreme Court adopted this new appendix 11/25/2020.
- Waiver/Fine Schedule, Ch6Art14App6, the Supreme Court adopted the amended appendix on December 9, 2020. The revision changed the statute numbers under the “Nebraska Department of Agriculture” section.
- Nebraska Juror Qualification Form, Ch6Art10AppA, the Supreme Court adopted the amended appendix on October 28, 2020 to become effective on January 1, 2021. The following changes made pursuant to LB387. Change to part I, number 1 from 65 to 70 years, number 4 wording. Part III, number 12, changed wording from “case” to “pending jury trial in any court”, number 13 changed from “criminal offense potentially punishable by one year or more of imprisonment” to “felony.” Clerks use only section, number 2A changed from 65 to 70, number 2C changed from “criminal” to “felony.”

- Two PDF versions of PowerPoint tutorials were uploaded and are accessible by the public. These are a resource to assist users in troubleshooting potential challenges in use of PDF forms. The introduction on the Master Forms Listing now includes links to these documents.
