• Removed date and signature line for “signed in my presence” and the Statement of “Execution before a judge or clerk is not necessary…” per the Supreme Court.
  • Ch6Art14App1 (CC 4:1), Plaintiff’s Claim and Notice to Defendant (Small Claims).
• Decommissioned, should only be using the appendix approved by Supreme Court :
  • CC 15:4, Personal and Financial Information of Parties for Estates and Trusts (Based on Ch 6 Art 14, App 8).

County Court and District Court:

• Added space for courtroom number. CC 13:24, Request for Transcript or Bill of Exceptions.
• Removed "STATE of NEBRASKA" from Plaintiff line to use in civil cases as well at request of Val Bendixen. CC 6:7.1, Record of Copy Provided Litigant in Forma Pauperis.
• Uploaded to Master List temporarily until the form is available or revised in the Court Reporting Personnel in the Portal.
  • CC 9:93, Estimate for Bill of Exceptions (language revised per Justice Cassel).
  • CC 9:94, Court Reporter Certification of No Record.

FINANCE:

• The following forms had the mileage rate changed to the current rate effective 1/1/2022.

  • HR 2:03, Statement for Payment of Substitute Reporting Services.
  • HR 2:05, Expense Reimbursement Request.

HR:

• Changed the W-4 to the 2022 version.
  • HR 1:07, New Employee Documents – Required by first day
  • HR 1:07C, New Employee Documents – Court – Required by first day

MISC:

• Form revised for content and spacing. AD 2:22, AOCP PPE Request Form.
• Number of District Court Judges changed. AD 2:24, Flowchart, Nebraska Court Structure Process of Appeal.

Trial Court Services works with Judges, Clerks of District Court, Clerk Magistrates and the Administrative Office of the Courts to provide forms to improve efficiency and consistency in the Courts and Administrative Offices.
