• CC 13:24   Information to Support Transcript or Bill of Exceptions – reformatted and renamed as instructed.
• CC 6:7.1 Record of Copy Provided Incarcerated Defendant in Forma Pauperis. - Removed "Exhibits" from options as there has to be a motion.  Removed the "Court Reporting Personnel" in the signature block as only the Clerk sends copies of the Transcript of Pleadings and Bill of Exceptions.  Note: the Court Reporting Personnel would send the Transcript of Proceedings. Made other slight changes per Amy Prenda and Valorie Bendixen.
